Does Medicare cover home health care in Virginia?

What Medicare pays for at home, who qualifies, what it won't cover, and where Virginia Medicaid and other programs fill the gaps.

Short answer: yes, for part-time skilled care, if certain conditions are met. Medicare does not pay for long-term help with daily living on its own. Here's how the rules work for Original Medicare, and what to check if you're on a Medicare Advantage plan.

Who qualifies

According to Medicare's home health coverage rules, you generally qualify if all of these are true:

  • You're under the care of a doctor, and your care follows a plan your doctor reviews regularly.
  • A doctor certifies that you need intermittent skilled nursing, physical therapy, speech-language pathology, or continued occupational therapy.
  • A doctor certifies that you're homebound — leaving home takes considerable effort, and you leave only infrequently or for medical care, religious services or adult day care.
  • The agency is Medicare-certified.

Your doctor or another allowed practitioner must also have a face-to-face visit with you related to the reason for home care, within a set window before or after care starts.

What Medicare pays

For eligible patients, Original Medicare pays for covered home health services in full — there's no deductible or coinsurance for the visits themselves. Durable medical equipment such as a walker or hospital bed is different: you typically pay 20% of the Medicare-approved amount after your Part B deductible.

Covered services can include part-time skilled nursing, therapy, medical social services, and part-time home health aide services when you're also getting skilled care.

What it won't pay for

  • 24-hour care at home
  • Meal delivery
  • Homemaker services like shopping and cleaning, when they're the only care you need
  • Personal care like bathing and dressing, when that's the only care you need

That's why many families combine Medicare home health with privately paid home care. TideCare explains typical private-pay pricing in its guide How much does home care cost?

Medicare Advantage plans

Medicare Advantage plans must cover the same home health benefits, but they can require prior authorization and use a network of agencies. Call your plan before choosing an agency, and ask the agency whether it's in network.

Other ways to pay in Virginia

  • Virginia Medicaid (Cardinal Care): Eligible adults may receive personal care through the Commonwealth Coordinated Care Plus waiver, including a consumer-directed option where a family member can be the paid caregiver.
  • VA benefits: Veterans may qualify for homemaker and home health aide services or Aid and Attendance payments.
  • Long-term care insurance: Check the elimination period and whether the policy requires a licensed agency.
  • Area Agencies on Aging: On the Southside, Senior Services of Southeastern Virginia can explain local programs; on the Peninsula, contact the Peninsula Agency on Aging.
Coverage rules change. Confirm eligibility with Medicare, your plan, or the agency's intake coordinator before care begins.
